Varicocele Grades

Varicocele is graded from 1 to 3 based on severity. Grade 1 is the smallest and is only palpable during Valsalva maneuver. Grade 2 is palpable without Valsalva. Diagnosis

Diagnosis of varicocele requiring surgical treatment involves:

  • Physical examination in standing and lying positions to assess vein dilation
  • Valsalva maneuver during examination to detect venous reflux
  • Scrotal Doppler ultrasound to confirm venous dilation and reflux

Surgical Procedure

Microscopic varicocelectomy is performed under general or spinal anesthesia using a surgical microscope for magnification. A small incision is made near the groin or in the subinguinal region. The spermatic cord is identified, and under microscopic vision, the abnormal veins are ligated while arteries, lymphatics, and the vas deferens are carefully preserved.

The microscopic varicocelectomy procedure involves:

  • Administration of general or spinal anesthesia
  • Making a 2 to 3 cm incision at the subinguinal or inguinal position
  • Delivering the spermatic cord out through the incision
  • Using an operating microscope at 6x to 25x magnification
  • Identifying and isolating each internal spermatic vein under magnification
  • Ligating all abnormal veins using clips or sutures
  • Preserving testicular arteries, lymphatics, and the vas deferens
  • Closing the incision in layers with absorbable sutures

After the Surgery

Recovery after microscopic varicocelectomy is relatively quick. Most patients are discharged on the same day. Mild scrotal swelling and discomfort are expected for a few days. Patients should avoid strenuous activity and sexual intercourse for 2 to 4 weeks. Semen analysis is repeated 3 to 6 months after surgery to assess improvement in sperm parameters and fertility outcomes.
