Causes

Causes of varicocele requiring open surgical repair include:

  • Absence or dysfunction of venous valves in the spermatic veins
  • Increased pressure in the left renal vein causing reflux into the spermatic vein
  • Anatomical compression of the left spermatic vein between the aorta and superior mesenteric artery
  • Elevated scrotal temperature impairing sperm production
  • Hormonal disruption due to chronic venous congestion

  • Dull, aching scrotal discomfort that worsens with prolonged standing

Varicocele Grading

Varicocele is classified into three grades. Grade 1 is only detectable by Valsalva maneuver. Grade 2 is palpable without Valsalva. Grade 3 is visible to the naked eye. Open varicocelectomy is most often recommended for grade 2 and grade 3 varicocele. Grade 1 may also be treated if associated with infertility, testicular atrophy, or significant chronic pain.

Who Needs Open Varicocele Surgery?

Open varicocelectomy is recommended for men with symptomatic grade 2 or 3 varicocele, men with varicocele-related infertility, adolescents with testicular growth retardation, and patients for whom laparoscopic or microscopic approaches are not available or suitable. The decision is based on clinical grade, semen analysis, symptoms, and the overall health of the patient.

Diagnosis

Diagnosis of varicocele before open surgery includes:

  • Physical examination in standing position with Valsalva maneuver to assess grade
  • Scrotal color Doppler ultrasound to confirm venous dilation and reflux
  • Semen analysis including count, motility, and morphology
  • Hormonal evaluation including testosterone, FSH, and LH levels
  • Testicular volume measurement by ultrasound to detect atrophy
  • Renal ultrasound in cases of right-sided or bilateral varicocele

Surgical Procedure

Open varicocelectomy is performed under general or spinal anesthesia. The most common approach is the inguinal technique, where an incision is made in the groin region. The spermatic cord is exposed and the dilated veins are identified and ligated while the artery and lymphatics are preserved. The procedure takes 30 to 60 minutes and may be done as day-care surgery.

Open Varicocelectomy Steps

The open varicocelectomy procedure involves:

  • Administering general or spinal anesthesia
  • Making an incision at the inguinal region or lower abdomen
  • Exposing the spermatic cord through the incision
  • Identifying the dilated internal spermatic veins using loupes or naked eye
  • Ligating all identified abnormal veins using sutures or clips
  • Preserving the testicular artery, lymphatics, and vas deferens
  • Closing the incision in layers with absorbable sutures
  • Applying dressings and a scrotal support

After the Surgery

Recovery after open varicocelectomy typically takes 2 to 4 weeks. Patients are generally discharged the same day or after one night. Light activities can be resumed within 1 week. Strenuous exercise, heavy lifting, and sexual intercourse should be avoided for 3 to 4 weeks. Semen analysis is repeated at 3 and 6 months post-operatively to assess sperm improvement.

Complications of Open Varicocele Surgery

Potential complications of open varicocelectomy include:

  • Hydrocele formation if lymphatic vessels are inadvertently ligated
  • Varicocele recurrence if all dilated veins are not identified
  • Testicular atrophy due to accidental ligation of the testicular artery
  • Wound infection or hematoma at the incision site
  • Scrotal swelling and bruising in the early recovery period
  • Nerve injury causing numbness in the scrotum or inner thigh
